    Julia Boothe, MD has transformed care in her rural Alabama community by making high-quality care management accessible to all her patients, while creating a significant revenue stream and operational efficiencies for her practice. In this session, Dr. Boothe will share a case study from Pickens County Primary Care, joined by co-presenter Zach Mortensen. They will explore how to re-envision care management for whole patient populations using fee-for-service programs like Chronic Care Management (CCM) and Advanced Primary Care Management (APCM), and how to lead the operational change needed to deliver sustainable, between-visit care. Attendees will learn why this approach benefits both primary and specialty care, how to ensure financial sustainability, and how care management can serve as a foundation for long-term population health.

Prior to joining Phamily, Zach founded and led an advisory practice that served digital health, medical device, clinical research, population health, and health plan clients. Zach is a graduate of The Wharton School with an MBA in Finance, and Arizona State University with a BSE in Computer Systems Engineering.

    Dr. Julia Boothe is a Board-Certified Family Medicine Physician who practices Rural Medicine in Pickens County Alabama. Her primary clinic is in Reform and she has multiple School Clinics she oversees in the Pickens County School System. She has been in Pickens County since completing her Family Medicine Residency as Chief Resident at Tuscaloosa Family Practice Residency in 2005. She graduated from the University of Alabama School of Medicine in 2002 and completed her Masters of Public Health from UAB in 2003. Dr. Boothe is past president of the Medical Association of the State of Alabama as well as her specialty society the Alabama Academy of Family Physicians.
