Julia Boothe, MD has transformed care in her rural Alabama community by making high-quality care management accessible to all her patients, while creating a significant revenue stream and operational efficiencies for her practice. In this session, Dr. Boothe will share a case study from Pickens County Primary Care, joined by co-presenter Zach Mortensen. They will explore how to re-envision care management for whole patient populations using fee-for-service programs like Chronic Care Management (CCM) and Advanced Primary Care Management (APCM), and how to lead the operational change needed to deliver sustainable, between-visit care.
